Practices begin! Make sure you have all your forms and paperwork submitted, so you are allowed to practice.

Forms & Tests for ALL Student-Athletes

All student-athletes should have received an email from @ACSathletics. This email has important information you need to complete. Please be sure to check your junk folder if you don’t see it. Some of you will get the request to your personal email address and some to your university email, so please check both.
Login to your account, or if it’s your first time logging in, create an account. Your user ID number is your student ID number. Once logged in, all forms listed on the dashboard need to be completed. To be allowed to practice and compete, all forms must be filled out.

Doctor Physical Forms

You will also need to have a doctor complete and sign a physical form.  Download a copy of the doctor physical form here. The physical must be dated after June 1 of this year and must be handed in to Kelly Fry in the Athletics Office. You can email her or drop it off before your first practice.

Sickle Cell Test Results

For those of you who are freshmen or transfer students, you must send a copy of your sickle cell test result to Kelly Fry. This is a newer NCAA mandate (effective August 2022), stating that all new athletes must provide the Athletic Department with sickle cell test results done at birth. If you are unsure if you got this test done at birth, ask your hospital or doctor for the results.  If you did not get your sickle cell test done at birth, you will need to schedule an appointment to get this done prior to the first practice. This is a blood test that can be done rather quickly.

Concussion Testing

Watch for an email from CSU’s athletic trainer for instructions to complete pre-season concussion testing prior to your first practice.

More Information

Please note that you will NOT be allowed to practice or compete until ALL paperwork is done and on file. If you have athletics questions, please reach out to Kelly Fry.
